Remove both outside bolts with a 5mm allen key then remove the wheel.Use a 17mm spanner and undo one of the nut on one side only. The axle will the slide out. Use a punch or screwdriver to knock out the bearings on both sides.
To refit push the bearings in both sides, make sure you have one of the 17mm nuts and the spacer on one side of the axle and slide through the bearings. Place the spacer on the other end of the axle and screw on the 17mm nut. When doing up the nut you need to make sure you have the same amount of thread protruding on each side.
These nuts should not be tighten too much as this will put too much pressure on the bearings, you may need to hold one side whilst you tighten the other. Slide the wheel back on to the forks and place the tighten the 5mm allen key bolts which screw into the end of the axle.
